Black seed oil and asthma: what the evidence says

Small or mixed human studies suggest black seed added to usual asthma treatment may improve symptom control and some lung-function measures, but every randomized trial had fewer than 100 people and lasted three months or less.

3/5 Limited

Some human evidence: small trials, trials on a related outcome, or population studies that disagree. How we score

Four small randomized trials have tested black seed in people with asthma, always on top of their normal treatment: oil capsules (80 people, 4 weeks, double-blind), seed powder (76 people, 3 months, single-blind) and a boiled seed extract (29 people, 3 months). A 2020 meta-analysis that pooled four trials found better asthma-control scores and FEV1, but not peak flow.

The best-designed trial is also the most modest: on black seed oil the Asthma Control Test score was 21.1 against 19.6 on placebo after four weeks, blood eosinophils (allergy cells) fell, and lung function (FEV1) did not change significantly. A 2026 critical review called the clinical importance of that 1.5-point gain uncertain.

In a single-dose study in 15 people, a boiled seed extract opened the airways within 30 minutes, but less than the asthma drugs theophylline and salbutamol. Nothing here suggests black seed can replace an inhaler, and no trial lasted longer than three months.

This is close to a 4 on our scale: there is one positive double-blind trial, but it was small and short, a quarter of its participants dropped out, and its lung-function result was negative. Larger, longer trials have not been published; a children's respiratory-allergy trial registered in Indonesia in 2022 has no results, and a 90-child asthma trial from Tanta University (NCT07315555) is registered but not yet recruiting.

We screened all 14 human-study records PubMed returned for this search and read every abstract, plus a 2010 airway study the search classed as a lab paper. Five records were about other conditions or were surveys of herbal use; one, a 2003 case of asthma triggered by bird seed, was about a different plant also sold as 'black seed' (niger seed, Guizotia abyssinica).

ResultTypeWhat they didWhat they found
PositiveSystematic review / meta-analysis, 2020Meta-analysis of 4 randomized trials of Nigella sativa in asthma
Measured: Asthma Control Test, FEV1, peak flow, IL-4, interferon-gamma
Higher Asthma Control Test scores (standardized difference 0.50, 95% CI 0.11 to 0.88) and FEV1 (1.84, 0.07 to 3.60); no clear change in peak flow, IL-4 or interferon-gamma. PubMed 31892440
PositiveSystematic review / meta-analysis, 2025Systematic review of 23 clinical studies of Western herbal and anthroposophic medicines in asthma, no pooled numbers
Measured: Symptoms, lung function, immune markers, safety
Nine studies of sufficient quality, Nigella sativa among them, reported benefits on symptoms, lung function or immune markers; the authors ask for more rigorous trials. PubMed 40490159
PositiveRandomized trial, 2017
80 people
Randomized, double-blind, placebo-controlled; 500 mg oil capsules twice daily for 4 weeks on top of usual asthma treatment (NCT02407262)
Measured: Asthma Control Test (main outcome), lung function, blood eosinophils
Asthma Control Test 21.1 vs 19.6 on placebo (p = 0.044); blood eosinophils fell (p = 0.013); FEV1 rose 4 vs 1 points, not significant (p = 0.170). 10 of 40 dropped out in each group. PubMed 28093815
PositiveRandomized trial, 2017
76 people
Single-blind, placebo-controlled; 1 g or 2 g seed powder a day for 3 months with inhaled maintenance therapy, partly controlled asthma (Saudi Arabia)
Measured: Lung function, peak-flow variability, exacerbations, exhaled nitric oxide, IgE
FEV1 and FEF25-75 rose at 2 g/day; peak-flow variability improved at both doses vs placebo; fewer exacerbations at 1 g/day. Control-test and nitric-oxide gains were reported against baseline only. PubMed 28151459
PositiveRandomized trial, 2007
29 people
Randomized, placebo-controlled; boiled seed extract daily for 3 months, adults with asthma (Iran)
Measured: Symptoms, wheeze, lung function, medicine use
At 3 months all symptom measures differed from placebo (P < 0.01 to 0.001); inhaler, oral steroid and theophylline use fell in the extract group only. PubMed 17868210
PositiveRandomized trial, 2008
40 people
Randomized, double-blind, placebo-controlled; boiled seed extract for 2 months in veterans with lung damage from chemical weapons (a related lung disease, not asthma)
Measured: Respiratory symptoms, wheeze, lung function
At 2 months all lung-function values and most symptoms were better than placebo (P < 0.01 to 0.001); inhaler and steroid use fell. PubMed 18991514
PositiveClinical trial, 2003
152 people
Four studies of oil capsules (40-80 mg/kg a day) in allergic rhinitis, asthma and eczema (Berlin)
Measured: Self-rated symptom severity, IgE, eosinophils, cortisol
Self-rated symptom scores fell during treatment in all four studies; the abstract gives no comparison with a placebo group. PubMed 14669258
PositiveClinical trial, 2010
15 people
Single doses of boiled extract (50 and 100 mg/kg) compared with theophylline, lung function measured over 3 hours, adults with asthma
Measured: Short-term airway opening (FEV1 and other flow measures)
Both doses widened the airways within 30 minutes, but FEV1 and most flow gains were smaller than with theophylline, and smaller than with salbutamol at 30 minutes. PubMed 20149611
MixedReview, 2026Critical review of defined natural products as add-on treatments (respiratory allergy, bowel disease, diabetes, depression, insomnia)
Measured: Clinical importance of the asthma results
Restates the 1.5-point Asthma Control Test gain over placebo at 4 weeks, calls its clinical importance uncertain, and notes FEV1 did not improve significantly. PubMed 42653115

What people report

We read public first-person posts and counted every outcome, good or bad. 2 reports about this condition, read on 2026-10-28; 0 mention side effects.

  • 1Helped
  • 1No effect
  • 0Got worse
  • 0Mixed
  • 0Too early

Why these counts prove nothing on their own: people who post are not a random sample, most take other treatments at the same time, and nobody checks the diagnosis. We show them because they tell researchers what to test next.

  • Helped Daily black seed oil plus mullein tea cleared chest mucus; cut a preventer inhaler to once daily, stopped another, no reliever for five months. HealthUnlocked, about 2021 (Date shown only as '5 years ago'; mullein started at the same time; medication changes self-reported, unclear whether doctor-supervised.)
  • No effect Took kalonji oil and mullein during flares for years; flares always ended up needing prescription drugs; now controlled on a biologic. HealthUnlocked, about 2021 (Long-term, retrospective account; several herbs together, so no single-remedy test.)

PubMed returned 97 papers for this search on 2026-10-28. We read the human studies above ourselves. The rest are sorted by type automatically from PubMed's labels, which are sometimes wrong. We removed 6 papers that only share a word with this search (niger bird seed, obesity studies, a trial protocol, a survey of herbal use, an animal meta-analysis and a preclinical review).
