Black seed oil and blood sugar and type 2 diabetes: what the evidence says

Based on the evidence, black seed lowers blood sugar modestly, about half a point of HbA1c in pooled trials of people with type 2 diabetes or metabolic syndrome, though most trials were small and short.

4/5 Moderate

At least one well-run randomized trial in people on this outcome, with a positive result. How we score

Every meta-analysis we found, from 7 to 82 trials, reports lower fasting glucose and HbA1c (the three-month blood sugar average) with black seed: HbA1c about 0.45 to 0.7 points and fasting glucose about 10 to 21 mg/dL lower than in control groups. Insulin levels and insulin resistance mostly did not change.

Placebo-controlled trials are small: 43 and 50 people with type 2 diabetes for 8 weeks, 140 women with metabolic syndrome for 8 weeks. One older trial (123 people, powdered seed) found no significant difference. Head to head, black seed oil was worse than metformin in newly diagnosed diabetes and about equal to metformin or lifestyle change in prediabetes.

The honest weak points: heterogeneity between trials is very high (one analysis found I2 of 93-96%), most trials last 8 to 12 weeks, several are open-label or not truly randomized, and a 2025 umbrella review rated the reviews of black seed low to very low quality. Black seed did not reach the 'moderate certainty' group that ginger and turmeric reached in that review. No trial has tested whether it prevents diabetes complications.

Thymoquinone, black seed's best-known compound, has one human trial here: 50 or 100 mg tablets added to metformin lowered HbA1c by 0.4-0.5 points against 0.2 on metformin alone in 60 patients. That is a purified compound, not the oil.

If you take diabetes medicine, do not add black seed or swap a prescription for it without your doctor: the trials show it lowers blood sugar, so it may add to the effect of insulin or diabetes pills.

We screened all 66 human-study records PubMed returned and read 42 abstracts in full: 15 of the 28 systematic reviews (the others covered other herbs, animal studies or other outcomes) and all 27 trial records (one a protocol); the rest were surveys of herbal use, protocols or studies of other outcomes.

What people report

We read public first-person posts and counted every outcome, good or bad. 4 reports about this condition, read on 2026-10-28; 0 mention side effects.

  • 2Helped
  • 1No effect
  • 0Got worse
  • 0Mixed
  • 1Too early

Why these counts prove nothing on their own: people who post are not a random sample, most take other treatments at the same time, and nobody checks the diagnosis. We show them because they tell researchers what to test next.

  • Too early After a first 1,500 mg dose of oil, one fasting reading came in lower than usual; seed powder taken earlier had disappointed. Diabetes Daily forum, 2016-01-06 (A single glucose reading after one dose.)
  • Helped Taking black seed since 2012 with sesame and chia; reports no metformin, HbA1c 5.4, normal cholesterol, weight loss and fewer allergy pills. WebMD user reviews, 2021-05-29 (Several seeds and probably diet changes together; no before values given; stopping metformin is the poster's own account.)
  • Helped In the first week of gel caps, joint pain and inflammation eased and glucose fell enough to lower insulin doses. WebMD user reviews, 2019-12-03 (Only one week; insulin changes self-made; low blood sugar is a risk when combined with insulin.)
  • No effect Nearly five months of oil brought no weight loss, and blood sugar went up over the same period. WebMD user reviews, 2020-04-22 (Review text is cut off; it does not say the rise was caused by black seed.)
