Ashwagandha and stress and anxiety: what the evidence says

Based on the evidence, standardized ashwagandha root extracts lower stress and anxiety scores more than placebo in many small trials, though most are short, from India, and several were run by the extract makers.

4/5 Moderate

At least one well-run randomized trial in people on this outcome, with a positive result. How we score

This is ashwagandha's best-studied use. At least six pooled analyses of randomized trials found lower anxiety or stress scores than on placebo: a 2022 analysis of 12 trials and 1,002 people, a 2024 analysis of 9 placebo trials (Hamilton anxiety 2.19 points lower) and a 2025 analysis of 14 trials in people with diagnosed mental disorders all point the same way.

The weak points are consistent too. Reviewers rated the certainty of the evidence low, the trials disagree wildly in effect size (one 2026 analysis reports effects several times larger than any other), most last 6 to 12 weeks, and most were run in India with branded extracts (KSM-66, Shoden, Sensoril, Zenroot, AshwaSR), several by the companies that sell them.

Trials run in the US and Australia are more mixed: of six that measured stress, four found no difference from placebo (including a 12-week Australian trial of Witholytin and a 2026 US trial in schizophrenia), one was borderline and one was positive. A 2025 analysis found that cortisol falls reliably while perceived stress, pooled over 6 trials, did not change significantly.

One industry trial of Sensoril in 120 people with generalized anxiety disorder finished in 2012 and has never reported results (see Beyond Google). Unreported trials tend to be the disappointing ones, so the published picture may be too rosy.

Ashwagandha is not a tested replacement for prescribed anxiety treatment. If you take antidepressants, sedatives, thyroid medicine or immune-suppressing drugs, talk to your doctor first; stopping it abruptly has been linked to rebound symptoms in one case report.

We screened all 255 records PubMed returned and read 89 abstracts in full: every record PubMed tags as a human study (71) plus 18 trials PubMed had not labelled. We kept 33 that measured anxiety, stress or a closely related outcome in people taking ashwagandha alone.

What people report

We read public first-person posts and counted every outcome, good or bad. 16 reports about this condition, read on 2026-10-28; 6 mention side effects.

  • 7Helped
  • 4No effect
  • 1Got worse
  • 4Mixed
  • 0Too early

Why these counts prove nothing on their own: people who post are not a random sample, most take other treatments at the same time, and nobody checks the diagnosis. We show them because they tell researchers what to test next.

  • Worse After three to four months, a two-day stimulant-like episode then a crash; symptoms persisted after stopping at eight months. Side effects: stimulant-like episode, crash, palpitations, exhaustion. HealthUnlocked, 2026-04 (Long prior illness and many supplements make the cause hard to judge. Date approximate (six months ago).)
  • Mixed Over two to three months felt calmer and more focused, but had headaches and an irritable rebound hours after doses. Side effects: headache, irritability hours after dosing. LongeCity forum, 2014-05-28 (Also on a prescribed stimulant; one product was of unknown quality.)

  • Helped Anxiety largely lifted within two days of plain root powder; it returned when doses were missed and eased on restarting. Side effects: mild headache. LongeCity forum, 2013-02-17 (On two prescription drugs; later stopped the antidepressant abruptly, which complicates the picture.)
  • No effect About three months with no difference either when starting or after stopping. Mumsnet, 2026-09-19 (No dose given.)
  • No effect A high-strength product did nothing for them. Mumsnet, 2026-09-19 (One-line report.)
