Black seed oil and chemotherapy side effects: what the evidence says

Small or mixed human studies suggest black seed may ease some side effects of chemotherapy and radiation, such as fever, mouth sores and skin damage, but the trials were small and several had no placebo.

3/5 LimitedPromising, unproven

Some human evidence: small trials, trials on a related outcome, or population studies that disagree. What exists leans positive, but it has not had a fair test: no controlled trial on this outcome, or only small ones. How we score

Small randomized trials point to help with treatment side effects: fewer fevers during chemotherapy in 80 children with brain tumors (2.2% vs 19.3% of low-white-cell episodes), less radiation skin damage with a 5% gel in 62 women with breast cancer, fewer mouth sores with an oil rinse in 54 leukemia patients, and less heart and liver damage in two 40-child leukemia trials from one Egyptian hospital. None was large, and several had no placebo.

These trials measured side effects, not whether the cancer responded, so they say little about black seed as a cancer treatment.

Black seed can interact with cancer care: case reports describe a large bleed during chemotherapy with antiplatelet drugs, very low platelets around surgery, and bone marrow suppression in a leukemia patient. Tell the oncology team if you take it.

ResultTypeWhat they didWhat they found
PositiveRandomized trial, 2017
80 people
Randomized controlled study (no placebo): 5 g seeds a day during chemotherapy vs nothing, children with brain tumors (Egypt)
Measured: Febrile neutropenia (fever with low white cells), hospital stay
8 of 372 neutropenic episodes had fever on black seed vs 63 of 327 in controls (2.2% vs 19.3%, p = 0.001); median hospital stay 2.5 vs 5 days. PubMed 28349493
PositiveRandomized trial, 2019
62 people
Randomized, double-blind, placebo-controlled; 5% black seed gel on the skin during breast radiotherapy (Iran)
Measured: Radiation skin damage, pain, skin quality of life
Radiation dermatitis was less frequent with the gel; grade 2 reactions started later (35 vs 29 days); skin-related quality of life did not differ. PubMed 31780017
PositiveRandomized trial, 2019
54 people
Randomly allocated: black seed oil mouth rinse vs 'magic mouthwash' during 28 days of chemotherapy, acute myeloid leukemia
Measured: Mouth sores (oral mucositis), pain, swallowing
Mouth-sore severity was lower (area under the curve 5.9 vs 38.4, P < 0.05), with less pain and better swallowing. PubMed 31781612
PositiveRandomized trial, 2020
40 people
Randomized: black seed oil 80 mg/kg or placebo for a week after each doxorubicin dose, children with acute lymphoblastic leukemia (Tanta, Egypt)
Measured: Heart function on echocardiography
Systolic heart function fell less on black seed than on placebo; diastolic measures did not change in either group. PubMed 31964219
PositiveRandomized trial, 2015
40 people
Black seed oil 80 mg/kg/day or placebo for a week after each methotrexate dose, children with acute lymphoblastic leukemia (Tanta, Egypt)
Measured: Liver toxicity; remission, relapse, survival
Liver tests rose significantly only in the placebo group; the authors report better remission, relapse, death and disease-free survival, but no difference in overall survival. PubMed 25809628
PositiveClinical trial, 2019
60 people
Single-blind clinical trial: oil applied near the IV line twice daily during chemotherapy vs no treatment
Measured: Chemotherapy-related vein inflammation (phlebitis)
Phlebitis was less frequent at 60 and 72 hours and less severe at 12 and 72 hours with the oil. PubMed 31453801
PositiveSystematic review / meta-analysis, 2026Systematic review of 9 studies of antibacterial agents to prevent mouth sores from head and neck radiotherapy
Measured: Radiation-induced oral mucositis
Black seed showed potential benefit on mucositis incidence and severity; the evidence for the agents overall was called limited and inconsistent. PubMed 41731134
